New Patient Forms and Policies

If you wish to make a new patient appointment with our office, please fill out the form using the link listed below.

Please make sure you fill out the forms fully before sending them to us. Please include a copy of your drivers license

and any dental insurance card you have. Once you are done filling out your forms, you may email them to us at the email

listed at the bottom of the page, or, you can fax them to us at (409) 896-2690. If your new patient forms are not filled out

and received before your appointment date, we may have to reschedule or cancel your appointment.

Here at Dr. Harpers office, we take COVID-19 Seriously. If you have or are showing ANY symptoms of COVID-19, please

let our staff know before your appointment so we can schedule you at a better time. DO NOT come into our office knowingly

having COVID-19 and not inform our office ahead of time. To do so puts everybody at risk and you will be asked to leave.

If you are seeking to be a new patient with our office, please fill out the COVID-19 Pre Screening form and send it along with

your new patient forms.
